Community Health Centers of the Central Coast, Inc. (CHC) is a non-profit organization that operates a network of community health centers in California's Central Coast. It provides comprehensive, patient-centered medical home services to diverse and medically underserved populations, including low-income individuals, migrant and seasonal farmworkers, and the homeless. CHC functions as a safety net provider with over 31 state-licensed facilities, emphasizing accessibility to remove barriers to care. Its integrated care model includes primary medical services, dental care, and behavioral health support, addressing mental, emotional, physical, and spiritual health needs. The organization also offers specialized programs for homeless care and counseling.
